Medication Pick Up
If your child had medication here at shool, please stop by to pick it up by June 21st. Otherwise the medication will be destroyed.
Individual Health Careplans and Medication Administration Forms
The Nurse's Office will be sending out forms for the 2010-2011 school year at the end of June. If your student was on medication on a routine or as needed basis, or has asthma, diabetes, severe allergies or a history of seizures, you will be receiving the appropriate form in the mail. Please be sure to return the form to the health office prior to the start of the school year. If the issue is no longer a problem, please return the form stating that, as it is important we make the necessary changes in your child's record. If your child has any of these concerns and does not receive one of these forms, please let us know.
MISSION STATEMENT
